Preparation The Liturgy Request Form is to be completed immediately for liturgies at which a bishop, archbishop or cardinal will preside or participate. The Liturgy Preparation Form is to be completed and submitted no later than two months before the scheduled Mass. Notes for Submitting Liturgy Request Liturgy Preparation Submission Form Our preference for submitting this form is via email. The form can be submitted via email by the following process: 1. Click on the form in the preferred language and Save it to your workstation 2. Open the MS Word document and fill out fields which allow for user input 3. Re-save the file under a different name 4. Attach the completed document file to a new email message 5. Send the email to the address indicated on the form.
